Service Coordination (SC) Service Coordination is an individually designed service which provides primary assistance to the waiver applicant/participant in gaining access to needed waiver and Medicaid State Plan services, as well as other local, state, and federally funded educational, vocational, social, and medical services.
Qualifications:
Individual with a Bachelor’s degree must have one (1) years of experience providing case management/service coordination, information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors;
Associate’s degree must have two (2) years’ experience providing case management/service coordination, information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors.
Community Services Coordinator will monitor Medicaid status. Report to HRA and/or DSS as required. Monitor annual lease renewals, complete annual housing audits, and handle other tasks as assigned.

Community Integration Counseling (CIC) is a counseling service provided to the waiver participant who is coping with altered abilities and skills, a revision of long-term expectations, or changes in roles in relation to significant others.
Qualifications:
Individual with a Master of Social Work;
Individual with a Master of Psychology; and
must have, at a minimum, two (2) years of experience providing adjustment related counseling to seniors and/or individuals with physical and/or cognitive disabilities and their families to be considered qualifying experience.
Independent Living Skills Training (ILST) is individually designed to improve or maintain the ability of the waiver participant to live as independently as possible in the community. ILST assists in recovering skills that have decreased as a result of onset of disability.
Qualifications:
Individual with a Bachelor’s degree and three (3) years of experience completing functional based assessments of ADLs and IADLs, developing comprehensive treatment plans, and teaching individuals with disabilities and/or seniors to be more functionally independent; or
An individual with an Associate’s degree and five (5) years of experience completing functional based assessments of ADLs and IADLs, developing a comprehensive treatment plan, and teaching individuals with disabilities and/or seniors to be more functionally independent.
