Service Coordination (SC) Service Coordination is an individually designed service which provides primary assistance to the waiver applicant/participant in gaining access to needed waiver and Medicaid State Plan services, as well as other local, state, and federally funded educational, vocational, social, and medical services.
Required qualifications for this position include:
Health-Related Social Needs (HRSN) Care Manager will provide ongoing, coordinated support to participants experiencing housing instability, food insecurity, transportation barriers, interpersonal violence, and other social determinants of health. The Care Manager will assess, plan, and manage services through a mix of telephonic and in-person interactions, ensuring individuals receive the housing advocacy, resource navigation, and community support they need.

Community Integration Counseling (CIC) is a counseling service provided to the waiver participant who is coping with altered abilities and skills, a revision of long-term expectations, or changes in roles in relation to significant others.

Independent Living Skills Training (ILST) is individually designed to improve or maintain the ability of the waiver participant to live as independently as possible in the community. ILST assists in recovering skills that have decreased as a result of onset of disability.
