Welcome to the Social Care Networks at Heavenly Hands. As a dedicated partner to our community, we are proud to be approved providers for Long Island (HEALI), Westchester (HVCC), and The Bronx (SOMOS) Social Care Networks. Our mission is to connect individuals and families with essential support, resources, and services — including housing assistance and advocacy — that empower well-being, strengthen stability, and promote positive change.

We offer comprehensive housing advocacy for individuals facing housing instability or in need of guidance. We offer the following:

• Assistance completing housing applications and required documentation
• Advocacy with landlords, property managers, and housing programs
• Support navigating shelter systems and emergency housing resources
• Stabilization services to help clients maintain long-term housing
• Referrals to community-based housing programs and financial assistance
   

Our goal is to ensure every client has the support they need to secure and sustain a safe home.

• HEALI – Long Island Social Care Network Heavenly Hands is proud to partner with HEALI as an approved provider within the Long Island Social Care Network. Through this collaboration, we extend vital support and resources to individuals and families throughout Long Island.
• SOMOS – Bronx Social Care Network Our partnership with SOMOS allows us to serve as an approved provider in the Bronx Social Care Network. Together, we connect Bronx residents with high-quality care and support services tailored to their unique needs.
• HVCC – Westchester Social Care Network Our collaboration with HVCC enables us to serve as an approved provider within the Westchester Social Care Network. Through this partnership, we connect Westchester residents to high-quality care and personalized support services designed to meet their unique needs.
