Open Positions
NHTD Service Coordinator
Provide Service Coordination to individuals enrolled in the NHTD Medicaid Waiver Program. Develop service plans, coordinate providers, and advocate for participants.
TBI Service Coordinator
Provide Service Coordination to individuals enrolled in the TBI Medicaid Waiver Program. Support community reintegration and individualized goal planning.
Community Integration Counselor (CIC)
Provide counseling and support to help waiver participants build meaningful community connections, develop social skills, and engage fully in community life.
Independent Living Skills Trainer (ILST)
Deliver hands-on training to NHTD and TBI Waiver participants to develop the everyday skills needed to live safely and independently in the community.
Positive Behavioral Intervention Specialist (PBIS)
Provide evidence-based, person-centered behavioral intervention and support to NHTD and TBI Waiver participants to address behavioral challenges and promote positive outcomes.
